Title: Study The Outcome Of Neurosurgical Interventions For Psychiatric Disorders.

Abstract :
Aim - To study the outcome of stereotactic neuro-intervention consisting of Lesioning and Deep Brain Stimulation in Psychiatric illness.



Material And Methods – Patients presenting to the Neurosurgery OPD of JHRC Mumbai suffering from MDD, OCD or TS and categorised as refractory to standard treatment were included in the study. Thorough evaluation and counselling of the patient and relatives was done. Patients were offered Lesioning/ DBS surgery at nuclei appropriate to their pathology and were followed up at 1 month, 3 month and 6 month interval with respect pre-operative and post-operative YBOCS,HAM-A, HAM-D, MADRS, YGTSS scores.



Analysis- Patient’s were grouped according to their respective pathology, treatment offered and the nuclei targeted. The pre-operative and post-operative scoring was done at periodic intervals. Descriptive analysis was done of the data accumulated.



Conclusion – We found that out of the nine patients undergoing treatment for OCD, six underwent lesioning and three underwent DBS. Six patients were in remission and three patients were responsive to treatments at the end of 6 months. In patients with MDD two were responsive at the end of 6 months whereas one patient was responsive to treatment. Both the patients were in remission at the end of 6 months however one patient developed wound infection and his entire DBS assembly had to be removed. Confusion was the most common complication in patients undergoing capsulotomy for OCD.
